The Catholic Charities of the Diocese of Houma-Thibodaux Assistance Network, herinafter refered to as "Charity Tracker", is a shared, computerized record keeping system that captures information about people experiencing a need for emergency services, including but not limited to assistance with utility bills, medications, rent/mortgage payments etc. Catholic Charities of the Diocese of Houma-Thibodaux (Administering Agency) administers Charity Tracker on behalf of participating agencies of the Charity Tracker Assistance Network, including Catholic Charities (Participating Agency).

I authorize Catholic Charities of the Diocese of Houma-Thibodaux as a Charity Tracker Participating Agency, to share my basic, identifying and non-confidential service transactions/information with other CharityTracker Participating Agencies. I further authorize Catholic Charities (Participating Agency), as a Charity Tracker Participating Agency, to share my dependent's basic, identifying and non-confidential service transactions/information wiht other CharityTracker participating agencies.

This Release of Information will remain in effect for 3 years from the date of submission.
